BCNC collects personal information from patients in the course of registration for a new service offered by BCNC.

HOW DOES BCNC USE PERSONAL INFORMATION?

BCNC’s primary purpose in collecting personal information of its patients is to enable BCNC to fulfil its service commitments by providing clinical management of a patient.

This may be used to:

• Verify your identity.
• Administer and manage your clinical account, including charging and billing and the collection of debts;
• Respond to specific requests for member information;
• Provide you with special information that are a sense of interest e.g. blog or newsletter.
• Storing and processing the individual's information.

ACCESS TO PERSONAL INFORMATION

An individual has the right to access and, if necessary, to correct personal information provided to and held by BCNC. For security reasons, a written request for access to Owner; Paul Bergamo may be required.

BCNC takes all reasonable steps to ensure that the personal information it collects, uses or discloses is accurate, complete and current. However, the accuracy of the information BCNC holds depends to a large extent on the information patients provide.
